Patient Registration

Please ask one of our receptionists for a registration form. The completed form should be handed to one of our receptionists who will check that you have given all the required information. Alternatively, you can download a registration form from this site. A proof of address will be required when registering at the Practice.

After registering with our practice, you will be invited to attend the surgery for a new patient check. This will be performed by one of our nurses and will take approximately 20 minutes. You will need to bring a urine sample to your appointment. Your blood pressure will be checked, your height and weight recorded and you will be asked various questions, all of which will help us to offer you the most appropriate care.
